Portrait Request Form


All portraits must be scheduled no less than 7 days in advance. If you have not received your confirmation email within 48 hours, please contact the Special Event Department as your session may not be secured.
  Once this form has been filled out, we will get back in touch with you to confirm the availability of your requested time and date.

*If you have questions before you book your session, please contact the Special Events Office at 615-354-6377. Pending availability, this form will book your request and charge your credit card accordingly. If the date you have requested is not available, your card will not be charged until you have arranged a new date with your photographer and the Special Events Office.
